MANAGEMENT

Guaranteed provides Enhanced Care Management (ECM) support for Medi-Cal members with complex health, behavioral, and social needs. These members navigate multiple care systems, including primary, specialty, dental, mental health, substance use treatment, and long-term services.

The ECM Medi-Cal benefit addresses the clinical and non-clinical needs of the highest-need members, which helps establish trusting relationships in the members' setting– at home, in a shelter, on the street, or wherever their setting, to meet their needs.

KEY FEATURES

The Enhanced Care Management (ECM) program is a Medi-Cal benefit in California designed to provide high-touch, coordinated care for Medicaid members with complex medical and social needs. It offers comprehensive, person-centered care management to help individuals navigate the healthcare system and access critical social services.

  • Target Population: Focuses on high-need groups such as individuals experiencing homelessness, those with serious mental illness or substance use disorders, high utilizers of healthcare services, and people with multiple chronic conditions.
  • Care Coordination: Assigns a dedicated care manager to work with members, helping coordinate medical, behavioral health, and social services.
  • Community-Based Approach: Services are often delivered in-person and in non-traditional settings (e.g., shelters, community clinics, homes). We meet members where they are!
  • Whole-Person Care: Addresses medical, behavioral health, and social determinants of health (e.g., housing, food, transportation).
  • Collaboration with Providers & Social Services:  ECM providers work closely with primary care providers, hospitals, behavioral health specialists, and social service organizations to ensure seamless care transitions and ongoing support.

Through community partnerships, Guaranteed ECM connects members with experienced care managers who coordinate comprehensive care beyond traditional medical settings, which helps vulnerable people access the care they need to improve their health outcomes and their quality of life.

A health and social services provider or a community based-organization may submit a referral for members.
